P.037 Neurofascin-155 IgG in acute-onset inflammatory polyneuropathy: possible predictor of relapse and recovery

چکیده

Background: IgG4 autoantibodies to neurofascin-155 (NF-155) have been described in a subset of patients with chronic inflammatory demyelinating polyneuropathy (CIDP). While reports suggest an acute onset is more likely than antibody negative CIDP, little literature exists around the subsequent course NF-155 positive cases that originally presented (AIDP) phenotype. Methods: Two male patients, ages 51 and 59, similar, <2 week histories lower extremity weakness. Patients were diagnosed AIDP treated IVIG. Following initial improvement, both relapsed. One patient was IVIG steroids improvement; however, he unable be weaned from without experiencing recurrence symptoms. The other not retreated. Testing for IgG sent. Results: first ultimately required Rituximab stable improved spontaneously. Both later had tests antibodies. Conclusions: our AIDP-like presentations, followed by relapsing excellent eventual recovery. This result, along limited available cases, presentation, could marker disease recurrence, but do necessarily predict poor outcome.

Paranodal dissection in chronic inflammatory demyelinating polyneuropathy with anti-neurofascin-155 and anti-contactin-1 antibodies.

OBJECTIVE To investigate the morphological features of chronic inflammatory demyelinating polyneuropathy (CIDP) with autoantibodies directed against paranodal junctional molecules, particularly focusing on the fine structures of the paranodes. METHODS We assessed sural nerve biopsy specimens obtained from 9 patients with CIDP with anti-neurofascin-155 antibodies and 1 patient with anti-contac...

Characterization of IgG4 anti-neurofascin 155 antibody-positive polyneuropathy

OBJECTIVE To investigate anti-neurofascin 155 (NF155) antibody-positive chronic inflammatory demyelinating polyneuropathy (CIDP). METHODS Sera from 50 consecutive CIDP patients diagnosed in our clinic, 32 patients with multiple sclerosis, 40 patients with other neuropathies including 26 with Guillain-Barré syndrome (GBS)/Fisher syndrome, and 30 healthy controls were measured for anti-NF antib...

Acute-onset chronic inflammatory demyelinating polyneuropathy: An electrodiagnostic study.

INTRODUCTION Acute-onset chronic inflammatory demyelinating polyneuropathy (A-CIDP) is an increasingly recognized CIDP subtype. Differentiating A-CIDP from Guillain-Barré syndrome (GBS) is challenging but important, because there are different treatment outcomes. METHODS We report 3 patients with A-CIDP who were initially diagnosed with severe GBS but were later confirmed to have CIDP based o...

Objective The objective of this study is to investigate whether chronic inflammatory demyelinating polyneuropathy (CIDP) and its subtypes differ in their type 1 T-helper (TH1) cell response against nodal/paranodal neurofascin (NF186, NF155) as well as myelin protein zero (P0 180-199) and myelin basic protein (MBP 82-100). Methods Interferon-gamma (IFN-γ) enzyme-linked immunospot assay was use...
